Join Our Surgery

We are always happy to accept new patients who live in our practice area to join our practice. Everyone who is a resident in the UK (living in the UK for more than 6 months) is entitled to register with a GP.

If you wish to register you will need the following documents originals - no photocopies:

  1. Photo ID (passport, driving license)
  2. Utility or council tax bill verifying your address (Water, Gas, Electricity, Council tax, Electoral Register Notification- under certain circumstances we will accept bank statements)
  3. NHS Medical Card (if you have misplaced your medical card or were not registered with a GP in the UK before, you can print out and complete the Registration Form - link below -and bring it to the surgery. Please ensure that all details are completed to avoid delays with registration)

We will take copies of the first two documents and they will be stored in your medical record and are therefore protected under the Data Protection Act. The Medical Card or pre-registration form is required to confirm your details with the NHS record service.

Reception will then hand you a medical questionnaire to complete and bring back when you attend your registration medical appointment.

Due to the fact that we have to take copies of your original documents, you will have to attend the surgery and also book a registration medical appointment with our nurse.

To help us in dealing with your registration fast and efficiently, please do not visit on Mondays or before 10.00am with all your documents as these times are very busy at reception.

Please note that if you fail to attend your registration medical appointment, we will cancel your registration with the practice.
