We are always happy to accept new patients who live in our practice area to join our practice.
If would like to register online, please only use the online registration form.
If they'd like to register in person, please print and fill out the PRF1 form and bring it to the surgery, or ask the surgery for one to fill out whilst on site.

If you wish to register you will need the following documents originals - no photocopies:

Current signed passport

Photographic registration cards

Current UK or EEA photocard driving licence

Residence permit issued by the Home Office

National identity card bearing a photograph of the applicant

Freedom pass bearing a photograph of the applicant

proof of address:

Utility bill (gas, electric, satellite television, landline phone bill) issued within the last three months

Local authority council tax bill for the current council tax year
-
Current UK driving licence (but only if not used for the name evidence)

Bank, Building Society or Credit Union statement or passbook dated within the last three months

HMRC self-assessment letters or tax demand dated within the current financial year
If you were registered under NHS before, please provide your NHS number as it makes it easier to identify the patients on the system.
We are part of a safe surgery and it is not mandatory to show a proof of ID and address but as per the policy we would ask patients if they can provide this.
To help us in dealing with your registration fast and efficiently, the reception will appreciate if you visit between 11:00am to 6:00pm from Monday to Friday.
Please note that if you fail to attend your new patient registration check, we will cancel your registration with the practice.
