Introduction to Online Access:
Online access for patients is a significant advancement in primary healthcare since it enables people to take a more active role in managing their own health. By using application like the NHS App, patients can check & cancel appointments, order their repeat prescriptions, and view their medical records conveniently. Patient empowerment, more transperency, and improved patient provider communication are all facilitated by this access.

To gain access to your account, use this online access request form, to request access to your records on the NHS app, and you can email your proof of ID and address to drkashyap.practice@nhs.net. Alternatively, you can also visit the reception between 10:00am to 6:00pm to collect a hardcopy.

Cancelling or changing an appointment
If you are unable to attend your appointment, please let us know as soon as possible so that the appointment may be given to another patient. You can check & cancel your appointment by:
- Calling us on 0208 918 0560
- Visiting the surgery and speaking with a receptionist - Check our opening times
- If you receive a text message reminder about the appointment you can also cancel your appointment through text message.
To help avoid delays
- Please arrive on time for your appointment. If you are running late, please contact the surgery reception on 020 8918 0560
- Please check in upon arrival at reception.
- Prioritise your clinical concerns and make a separate appointment for less concerning problems
- Please cancel your appointment in advance if you are unable to attend, it would benefit other patients in waiting
- Please ensure you have the correct mobile number on file, hence, if you change your number, please contact the reception to change your contact number
In a life-threatening emergency - dial 999
Examples of medical emergencies include but are not limited to chest pain, difficulty breathing, loss of consciousness, severe allergic reactions, severe blood loss, fitting, severe burns or injuries.
For urgent care - dial 111
111 is free and available 24 hours a day. 111 is a non-emergency service which offers healthcare assessments and can signpost you to the most appropriate care for your condition which can include: self-care advice, face to face appointments or the emergency department.
Walk in Centres / Out of hours service:
Routine out of hour appointments are provided in the evening and weekends at the following locations. You will be told which location you need to travel to when your appointment is booked:
Barking Hospital:
Monday to Friday: 6:30pm to 10:00pm
Saturday: 10:00am to 8:00pm
Sunday: 10:00am to 8:00pm
Broad Street Medical Centre:
Monday to Friday: 6:30pm to 10:00pm
Saturday: 9:00am to 5:00pm
The same locations also provide a walk in GP service.
Together First Enhanced Access Service:
Barking and Dagenham GP Federation provide enhanced appointments across the borough, over 2 sites that is open to patients outside of normal GP opening hours.
This service allows patients to see a clinician weekday evening, and on weekends.
Our appointments are available in hub sites at the following sites:

Days: Monday to Friday (6.30pm to 9.30pm);
Saturday & Sunday (9.00 to 5.00pm)
Site: Broad Street, Morland Road, Dagenham Essex RM10 9HU

Saturday (9.00 to 5.00pm)
Home Visits
If you need a home visit contact the surgery. A telephone consultation will be arranged. The clinician will then decide if a home visit is needed.
Remember that we can see several patients at the practice in the time it takes for one home visit. Patients should have their appointment at the surgery if they can.
Lack of transport is not a reason to request a home visit.
